There is virtually no country in the world with such a significant gap between rural and urban areas when it comes to the standards of healthcare as India. Approximately 70% of India’s population lives in rural areas, and virtually all of these people have extremely limited access to medical care. Thankfully, that is slowly changing. But thus far, to a certain extent, alternative medicine fills the gap in care for many people. Expats who will be working and living in rural areas need an insurance policy that accounts for emergency transportation to urban facilities.

How Routine Doctor Visits Work

Many doctors do rounds at the local hospital by day and run their own practice at night. Public hospitals also usually have a duty doctor or family doctor on call to have non-urgent concerns addressed. However, at many private facilities, a family medicine clinic sees patients for non-urgent concerns during regular business hours. Doctors are also usually very responsive to address concerns over the phone and even sometimes over apps like WhatsApp.

Overview of India’s National Health Insurance System

Technically, India has a system of universal healthcare. The Indian constitution makes the provision of healthcare in India the responsibility of the state governments. They are tasked with taking care of nutrition, the standard of living, and public health. As many as 90% of the people in India use government-run hospitals, where care is free or very heavily subsidized. However, the system is fraught with challenges. The standard of care in government-run facilities ranges from basic to abysmal. For instance, medications that are supposed to be provided for free are often unavailable. Bribery is a common practice, a necessity to be seen sooner or treated better. Waiting times for basic appointments can take days or even weeks.

However, in fairness to India, there is also a lot to celebrate. Despite massive obstacles, India has managed to provide free healthcare – however basic – to a country of over 1 billion people. And, at present, huge changes are being rolled out to help India’s poorest citizens.

Understanding New Changes in India

A massive change is currently underway. In 2018, India introduced a new national health protection scheme. It’s called Ayushman Bharat , or the National Health Protection Mission. Under this plan, the poorest half of India’s population (which is more than 500 million people), will receive $7,200 in annual coverage to apply towards care at private hospitals. For people used to the health costs in places like the United States that might not seem like a lot of money. However, in India, the cost of medical procedures is much lower. For instance, gallbladder surgery at a private hospital could cost less than $500. As such, $7,200 would make a strong, measurable difference in the lives of India’s poorest citizens.

Another important development is the establishment of some 150,000 health and wellness centers or HWCs, to increase access points for medical care. This move means that India is closer to meeting its commitments of universal health coverage, a key part of the United Nations Sustainable Development Goals .

In the first year since implementation, the Ayushman Bharat program has reached nearly 4 million people. The resulting savings for the affected families is estimated at 1.6 billion dollars. However, critics allege that money is being spent recklessly, that there are significant gaps in the infrastructure which aren’t being addressed, and that there is an acute shortage of skilled workers.

Pros and Cons of Using the Local Health System

At present, virtually all forms of the public health system in India struggle to provide services in a timely manner. Public facilities are devoid of privacy and comfort. Patients often have to share beds and cleanliness is dubious. Expats should avoid public health facilities unless they have no other choice. It’s not just about comfort. In these overcrowded facilities, locals struggle for basic care. Expats who can afford to seek care elsewhere should do so in order not to drain much-needed resources from the public.

If you do use public health facilities, be aware that in addition to treatment costs, you will also likely pay a one-time registration fee. If you are admitted, you will also pay room charges. The different categories of patient accommodations often have optimistic names like “deluxe”, “deluxe with AC” or “super deluxe”. Choose the highest level you can afford to increase your comfort and privacy. And while it’s easier said than done when you aren’t feeling well, insist on reviewing your bill to ensure your room charges actually correspond with the accommodations you receive.

The contrast between public and private care in India could not be more pronounced. Urban centers in India have an extensive network of private hospitals and clinic which provide high-quality care. Standards are so high in fact that India is one of the popular destinations in the world for medical tourism. Expats can expect private, comfortable rooms with modern amenities, English-speaking doctors, specialists, nurses, and support staff, and even access to alternative treatments. While it can take days to see a doctor at a local public emergency room, private hospitals and clinics offer timely care, even for specialist referrals.

Do I Need Travel Insurance for India?

Coverage of Travel Health Insurance

An international travel health insurance policy provides coverage to you in the following situations

However, there is a limit on the amount for the coverage in the policy.

Trip Cancellation Coverage : If you are covered by a group travel medical insurance, and in case your trip or that of your travel companion or an immediate family member has to be cancelled before commencement due to a COVID-19 infection, the coverage reimburses the non-refundable and the unused amount of the hotel bookings, tickets, etc. But do note that these expenses will only be reimbursed if the bookings were made prior to being diagnosed with COVID-19.

Automatic Extension Benefit : If you are covered by our international travel medical insurance, then in case of a lockdown in the other country and due to the non-availability of an alternate mode of transport, your policy will be automatically extended by 7 days. Eligibility Criteria for Travel Medical Insurance Policy in India

In the case of a student travel insurance policy, you will have to be a student between the age of 16 - 35 years, travelling abroad for higher education.

International as well as domestic travel insurance plans are quite flexible and can cover up to 6 family members, including you. The age limit for a family member starts from 6 months of age with no upper age limit.

If you are travelling with non-family members or friends, each person will have to have individual travel insurance.

A senior citizen travel insurance policy is applicable for senior citizens starting from 71 years of age with no upper age limit. There are no restrictions such as medical tests or pre-existing conditions.

The rise in medical tourism can be attributed to factors like India's competitive pricing compared to Western countries, the availability of advanced treatments, and the country's diverse cultural and touristic appeal. Kolkata, in particular, has seen a growing trend in the IVF landscape, with several state-of-the-art fertility clinics offering comprehensive services to both domestic and international patients. Not only doctors, foreigners are also seeking affordable psychological and psychotherapeutic consultations in India. Mansi Poddar, a trauma-informed Psychotherapist gushed, “Medical tourism including mental health services are relatively cheaper and more easily accessible in India. For example, mental health appointments are faster in India. There is no hassle around insurance/ number of sessions/documentations etc. I work with various clients abroad and they say that scheduling appointments is easier and quicker, they have access to me on a regular basis, there is no insurance limit on sessions plus they don’t have to worry about permanent documentation. Also, culturally it’s easier to see a therapist who has partly lived outside the country and in the country.”

An unexpected medical emergency would be the last thing any student would want to come across.

Going abroad to study is a dream for countless Indian students. It’s a goal that requires years and years of hard work and dedication. The allure is not only of a better and world-class education. The idea of studying abroad brings with it an opportunity to experience a new culture and immerse oneself in it. It also comes with the promise of a bright career and a better quality of life in a developed country. Due to all these reasons, a large number of young Indians aspire to explore international shores for their higher education.

A testament to this fact is that there has been a huge increase in the number of Indians studying abroad — a nearly 70 per cent spike in 2022 as per the Union Education Ministry. And it is only likely to go here. More and more Indian youngsters will look to Europe, the US, Canada, Singapore, Australia and other countries in the coming years.

The cost to fund this dream

This dream of international education comes with steep costs. First, there is tuition which can cost tens of lakhs of rupees. Then there are costs of accommodation, other living expenses, and not to forget travel costs, which can be substantial. Families often exhaust their entire savings to send their children abroad. Some even borrow to help their children fulfil their dreams. And more often than not, even the students take educational loans to fund their studies.

In these circumstances, an unexpected medical emergency would be the last thing any student would want to come across. Already under tremendous financial stress due to the heavy cost of international education, such an emergency can rip their finances apart. And add to that the stress of navigating a complex and expensive healthcare system in an unfamiliar country.

The good news is that most colleges abroad understand these risks and make it mandatory for students to get medical insurance. However, most students opt for policies that are either provided by their colleges or are recommended by them. While these policies may be comprehensive, they may not be ideally suited for foreign students — Indians and others.

Why College-Provided Medical Insurance Might Fall Short

Policies provided or recommended by colleges abroad typically cover students for medical emergencies faced within the campus or the immediate surroundings. At most, they will cover the student within their country of residence. However, a student living abroad is not going to restrict himself/herself only to the campus. They are going to take this once-in-a-lifetime opportunity to explore.

During semester breaks, these students are going to explore neighbouring countries. At least once or twice during their course, they may also travel to India to see their families on important occasions like festivals, weddings in their families and so on. What happens if they face medical emergencies during these travels?

In most cases, college-provided policies do not cover the student in such situations. This will leave them in a vulnerable situation as they would have to meet the expenses out of their own pockets amid the already high expenses of their education.
